Please consider this a reminder that if you opened a Campaign Account prior to
December 31, 2012, you are required to submit a Campaign Treasurer's Report that is
due in my office by the close of business at 6:00 a.m. on Thursday, 7anuarv
10, 2013. The Report Code for the report would be "Q4" and it covers the period
10/01/2012 through 12/31/2012. If you have opened your account but not
collected any funds, you must submit a "Waiver of Report ".
I am out of the office on vacation and will return on Monday, January 7 If you have

Good afternoon Allan:
I just called you and left a message on your telephone regarding this subject. Please be advised that
I just reviewed your Termination Report and what I find is that your report is not complete since your
Total Monetary Contributions To Date and your Total Monetary Expenditures To Date do not
balance. You report contributions of $9,805.47, but you only account for $8,662.10. It appears
there is a $1,143.37 difference that needs to be accounted for.
You will need to do an "amendment" to the report in order to balance out the account. In
accordance with Florida State Statute, you have three days to produce an amendment to correct the
report. Therefore, it will be necessary for you to submit your amendment to my office by 3:15
p.m. on Monday, June 24, 2013. (I believe I said Tuesday on the phone, but it is Monday, June
