Although the money reporting is correct, there are some errors on the report that require an amendment to be filed. The errors on the 2014 G -3 Report are as follows: • On the Itemized Contributions Sheets, you need to include the occupations of the businesses that have contributed $100 or more. Some of them are missing. Also, Contribution Type is missing from one of the entries. • The Sequence Numbers are missing from both the Itemized Contributions and Itemized Expenditures. • On the Itemized Expenditures, the Sequence Numbers of the expenditures need to begin with #1. You cannot enter the check number. Also, on the expenditure to Mark Graphics, the date is missing as well as the Sequence Number. The "purpose" of the expenditure needs to be in the "Purpose" column and the "Expenditure Type" must be the code that is found on the back of the Form in the table. I have attached blank forms for you to use to prepare the amendment. The Division of Elections advised that you may not use Office Account funds for a re- election campaign; however, if you open an Office Account now and are successful in your re- election campaign, you would be able to take those Office Account funds and transfer them into a new Office Account. If you are not successful in your re- election campaign, you would then have to dispose of those Office Account funds. I hope this helps in your decision. 1
